El Big Data es una de las mayores revoluciones de la historia de la tecnología y la medicina no es ajena a esos beneficios. Por ello, desde Futurs ya se está trabajando en el desarrollo de plataformas digitales alimentadas por algoritmos y ML para crear un nuevo modelo sanitario digital.
Un ejemplo de aplicación de los modelos predictivos fue el algoritmo desarrollado con las herramientas de Azure Machine Learning de Microsoft y que, durante el peor momento de la pandemia, ayudó a determinar qué pacientes positivos por coronavirus podían desarrollar un empeoramiento que requiriera su traslado a la Unidad de Cuidados Intensivos.
“El Big Data en Sanidad tiene mucho más potencial que en otros sectores. La razón es sencilla: en el sector Salud se trabaja con muchas variables que, por pequeñas que sean, pueden cambiar el curso de una enfermedad de manera disruptiva”, afirma Francisco Manuel Aznar, Development Manager de Futurs.
Como explicaba González durante su intervención en el Forbes Talks, dedicado a la digitalización del sistema sanitario, “obviamente, la decisión clínica no era del algoritmo, pero su uso nos permitía hacer un uso más eficaz de un recurso que, en aquel momento, era tan escaso como las camas UCI. Y lo conseguimos, sobre todo, gracias a la interoperabilidad y a un procesado de datos eficaz”. Además, el algoritmo preveía qué pacientes con COVID-19 tenían más probabilidades de reingresar en los siguientes 30 días al alta.
Estos sistemas necesitan ser entrenados con una gran cantidad de datos, ya que ese volumen determinará la sensibilidad de los análisis: a más datos, mejores predicciones, ya que habrá más variables bajo estudio. La clave está en recopilarlos y procesarlos correctamente, ya que proceden de fuentes muy diversas.
Futurs tiene implementado un proceso muy concreto para continuar alimentando los diferentes algoritmos que ha desarrollado. “En los proyectos de algoritmos para prevenir úlceras por presión y de caídas, por ejemplo, volvemos a recopilar los datos cada seis meses y los mandamos a la máquina para ver qué ha pasado en ese tiempo. Dependiendo de la estadística y los datos que nos devuelva, volvemos a entrenar el modelo picando nuevas instrucciones o lo dejamos como estaba. De esta forma, identificaríamos variables que ya no aportan información relevante y meteríamos otras nuevas que nos trasladen los sanitarios”, explica Adrián Belso Garzas, Lead Data Scientist en Futurs.
La experiencia del grupo sanitario Ribera, que gestiona nueve hospitales en España y supera los 7.000 profesionales, permite crear una extensa base de datos médicos de pacientes con distintas patologías que sirven para alimentar y entrenar los algoritmos de estas plataformas digitales. Todos estos datos desembocan en la historia clínica electrónica desarrollada por Futurs, Cynara Care, una solución con características cognitivas que sirve para apoyar al personal asistencial en la toma de decisiones clínicas en tiempo real. Es una historia clínica modulable, pensada para cubrir desde necesidades específicas hasta dar respuesta global a la actividad clínica y administrativa generada en un clúster de salud, hasta el punto de incluir algoritmos de predicción.
Cuando se tienen estos datos, el siguiente paso es la planificación del enfoque asistencial de las personas, algo que se ve facilitado gracias a la tecnología, y que tiene en realidades como TruCare una experiencia tangible. Se trata de una plataforma que permite personalizar tratamientos médicos de forma automatizada gracias a la información previamente recopilada por dispositivos conectados, o bien aquella disponible en las grandes bases de datos que ya están creando los centros hospitalarios. Se produce, entonces, un itinerario que parte de un dispositivo que recopila datos y los envía a una nube, donde una Inteligencia Artificial los analiza, clasifica y archiva, extrayendo patrones que plataformas como TruCare utilizan para sugerir intervenciones concretas por parte del profesional médico.
“El core de TruCare es elaborar valoraciones estandarizadas en función del nivel de cronicidad del paciente para crear planes de cuidados según esa valoración”, explica Francisco Ballesta, coordinador de la Unidad de Gestión de Salud Poblacional de Grupo Ribera y Product Owner de Cynara Citizen. Utilizando los recursos que ya recopilaba Futurs, “fuimos capaces de recoger mucha información de la historia clínica electrónica y de la historia clínica de atención primaria; con esto segmentamos la población y la estratificamos en niveles de riesgo. Luego, TruCare nos permitió valorar a los pacientes en función de este nivel de riesgo y, según esta valoración, crear planes de cuidados personalizados y estandarizados para cada ciudadano”, destaca.
Todo ello permite contemplar un ecosistema digital que, bajo el concepto del hospital en remoto, crea un diseño modular infinito con diversas plataformas interconectadas entre sí, destinadas a recopilar y procesar datos de distinta índole. Un trabajo de innovación y transformación digital que se lleva a cabo con el objetivo de mejorar la vida de las personas prácticamente sin que se den cuenta, mientras siguen disfrutando de su día a día. La tecnología y la medicina trabajan aliadas en un segundo plano para que todo marche bien.
