Se trata de un proyecto pionero a nivel mundial desarrollado por profesionales españoles para mejorar la calidad de vida y el bienestar psicoemocional de los pacientes pediátricos con trasplante hepático y el de sus familias.
España sigue siendo por vigesimosexto año consecutivo el país líder mundial en trasplantes. El 80 por ciento de los trasplantes hepáticos pediátricos que se llevan a cabo en nuestro país se realizan durante el primer año de vida del paciente o con edades comprendidas entre el primer año de vida y los siete años. La vida de un bebé-niño cambia por completo después del trasplante, por lo que para acompañarlos durante todo el proceso de trasplante nace ‘Virtual Transplant Reality (VTR)’, un proyecto pionero a nivel mundial desarrollado por profesionales españoles para mejorar la calidad de vida y el bienestar psicoemocional de los pacientes pediátricos con trasplante hepático y el de sus familias.
La iniciativa, promovida por Erika Guijarro, psicóloga con amplia experiencia con este tipo de pacientes y especialista en tratamientos con Realidad Virtual, será desarrollada de forma conjunta por ella misma y un consorcio de entidades e instituciones como el Instituto de Investigación del Hospital Universitario La Paz, IdiPAZ, la Asociación HePA (Asociación Española de Ayuda a Niños con Enfermedades Hepáticas y Trasplantados Hepáticos) y como responsable tecnológico del proyecto el Centro Universitario de Tecnología y Arte Digital, U-tad.
Será en la ‘Unidad de Trasplantes Pediátricos’ del Hospital Universitario La Paz donde de forma pionera se llevará a cabo la implementación práctica de esta herramienta, al ser este centro hospitalario que más trasplantes pediátricos realiza en toda España y donde ya colabora la Asociación HePA.
El proyecto consiste en la creación de una plataforma tecnológica que incorporará y gestionará todas las experiencias en realidad virtual y realidad aumentada. En cada experiencia se utilizarán técnicas gamificadas, narrativas e interactivas, así como se crearán mundos inmersivos, adaptados a cada problemática. Estas tecnologías facilitan el entrenamiento y la adaptación de los usuarios a entornos aislados y controlados. Además, son de fácil manejo y se pueden ejecutar en diferentes tipos de dispositivos, según las necesidades de cada paciente.
“La realidad virtual se ha convertido en una gran aliada para los profesionales sanitarios y para todos aquellos que sufren algún tipo de patología, por la gran capacidad de inmersión y realismo que ofrece, cuestión si cabe más importante cuando hablamos de pacientes pediátricos (entre los 0 y los 18 años) donde se necesita más interacción con el entorno para conseguir mayores beneficios en el tratamiento al aumentar su motivación” afirma Erika Guijarro, directora – coordinadora de la iniciativa y psicóloga especialista en tratamientos con Realidad Virtual.
El proceso de trasplante en un paciente pediátrico (bebés, niños y menores de 18 años) supone una serie de consecuencias para el enfermo y su familia a nivel emocional, social, económico y laboral. A través del uso de la Realidad Virtual y de la Realidad Aumentada, VTR pretende paliar estas consecuencias, así como mejorar la calidad de vida y el bienestar psicoemocional, tanto del paciente como el de su entorno. Para ello se desarrollarán escenarios y experiencias virtuales, (que seguirán un proceso evolutivo durante las fases del trasplante) desde una triple perspectiva:
Prevención (anticipando diferentes escenarios): el objetivo es entrenar a los niños en aquellas situaciones estresantes o potencialmente traumáticas como estancias en UCI o pruebas diagnósticas aparatosas, que suelen producir mucha angustia y ansiedad en este tipo de pacientes. De esta manera, cuando llegue el momento serán capaces de controlar estos estados de ánimo, pudiendo hacer innecesaria la administración de anestesias evitando así el consiguiente riesgo para el paciente y contribuyendo a una reducción de costes para el sistema.
Distracción (terapias distractoras del dolor o de manejo del mismo): se pretende cambiar el foco atencional del paciente pediátrico para que se centre en los estímulos que le proporcionará esta herramienta y no en las pruebas médicas.
Concienciación (autocuidado y adherencia al tratamiento): se favorece la adherencia al tratamiento tanto de los pacientes como de su entorno (amigos, familiares, etc.) haciéndoles partícipes de las rutinas del niño trasplantado (pruebas médicas, ingresos, cirugías, estancias en Unidades de Cuidados Intensivos, medicación, rutinas de cuidado y precauciones, prohibiciones médicas, etc.).
Está demostrado que en el paso a la adolescencia de un paciente trasplantado se pierde el 30% de los injertos debido a que el joven deja de seguir su tratamiento, abandona su medicación, comienza a beber alcohol, etc. A través de estas experiencias interactivas se pretende que el círculo de amistades y familiares del paciente trasplantado hepático sea consciente de sus cuidados y le anime a mantener su autocuidado y la adherencia al tratamiento, ya que su medicación es su único seguro de vida.
La Paz, coordinador de ‘Transplantchild’ la Red Europea de Referencia (ERN) en el Trasplante Pediátrico
El Hospital Universitario La Paz coordina ‘TransplantChild’, la Red Europea de Referencia (ERN) de Trasplantes Infantiles, encabezando la única Red Europea de Referencia coordinada por España. Es una de las 24 Redes Europeas de Referencia (ERN) aprobadas por la Comisión Europea en cumplimiento de la Directiva de la UE sobre derechos de los pacientes en la asistencia transfronteriza.
Este hospital cuenta con uno de los equipos multidisciplinares de trasplantes más altamente especializado de Europa y es el único en España acreditado para hacer todo tipo de trasplantes infantiles (Programa de trasplantes). Desde enero de 1985 y hasta diciembre de 2016 han realizado un total de 1.799 trasplantes de órgano sólido y progenitores hematopoyéticos, con unos indicadores de supervivencia y de calidad equiparables a los mejores hospitales del mundo.
La Red TransplantChild está coordinada por la Dra. Paloma Jara y está constituida por 18 hospitales de once países: Alemania, Bélgica, Francia, Gran Bretaña, Holanda, Italia, Lituania, Polonia, Portugal y Suecia, además de España. Es una red virtual de colaboración e investigación que posibilitará un mejor conocimiento del trasplante pediátrico.
España, líder mundial en trasplantes
Recientemente la Organización Mundial de Trasplantes ha hecho públicos los últimos datos recogidos por el Registro Mundial de Trasplantes, y que sitúan a España como país líder mundial en trasplantes por vigesimosexto año consecutivo con 2.183 donantes y 5.261 órganos trasplantados en 2017. En total, se efectuaron 3.269 trasplantes renales, 1.247 hepáticos, 304 cardíacos, 363 pulmonares, 70 de páncreas y 8 intestinales. Actualmente, nuestro Modelo Español de Trasplante (MET) está siendo replicado en todo el mundo.
Por hospitales, es el Hospital Universitario La Paz en el que se realizan el 70% de los trasplantes pediátricos nacionales seguido por el Hospital Vall d’Hebron (Barcelona), La Fe (Valencia), el Reina Sofía de Córdoba y el 12 de Octubre de Madrid.
